The Children's Rehabilitative Services at FMC offers specialty pediatric outpatient clinics for Northern Arizona children with disabilities or chronic illnesses.

Children's Rehabilitative Services (CRS)/ Arizona Department of Health Services

What is CRS? CRS is a statewide-managed care program for children with chronic and disabling conditions, which have the potential for cure or significant improvement. CRS provides specialty medical care for these children so they can develop to their fullest potential. This is done by directly providing selected services and enhancing access to services needed by this population of children and families.

Is CRS new to Arizona? CRS has a long history in the state of Arizona. Originally, it served children with polio and other orthopaedic problems and as medical technology improved, its coverage was expanded to include other rehabilitative conditions.

Where are CRS services located? Arizona CRS hospital and clinic services are provided in Flagstaff, Phoenix, Tucson and Yuma with field clinics located in many outlying rural communities.

How are services paid for?
There are three primary sources of funding designated for CRS:

  • State appropriations: The Arizona Legislature appropriates funds for CRS.

  • Federal funds: These include the Maternal and Child Health (MCH) Block Grant and other federal revenue sources designated for use by the AHCCCS program.

  • Patient/third-party payment: Some CRS patient families are billed for services on a sliding-fee scale according to their ability to pay. Some CRS patients have private insurance coverage that helps pay for certain services.

  • www.ahcccs.state.az.us: - Patients with certain medical conditions are often eligible for CRS services.

Are there eligibility criteria for CRS services? CRS programs are available to Arizona residents under 21 years of age who have an eligible physical disability, chronic illness or condition that could become disabling. To determine eligibility, you must submit a completed CRS application, which will be reviewed by our medical director. If it is determined that you are not eligible for CRS, your child may still be provided treatment through Children’s Health Services (CHS).
